We have seen the ITV problem reported quite a bit; they seem to be a great deal more capable at playing us adverts which are live than feeding us the subsequent on demand programme that we came for. Quite often, ITV will fix these issues, which are temporary, if rather too frequent.
And they can relate to a particular programme and not others, so it is useful to specify which programme when, when talking to ITV or your ISP.
